Need Puppy Potty Help

Olive is 9 weeks old and she has been with our little family for almost 1 week now. She was amazing with going potty outside the first few days. Sure she would have the occasional "piddle" inside until I figured out the whole, take her out after sleeping, eating and playing concept. But for the most part she's really good about going outside. She also does really good in her crate. I don't go to bed until close to Midnight and "Mommy" gets her up at about 4:30am. Never had an accident in her crate ever.

This morning, we woke up to find she actually pooped in her crate. She'd never done that until today and we felt so guilty but she didn't give us any of the normal potty signals. (whining, whimpering, rattling the cage)

We gave her a nice warm bath and she seems to be doing "ok" emotionally but since the crate incident this morning, she's had 2 poops INSIDE the house. I don't really know what changed for us. (She's never done that before)

After reading a few posts, I want to make it clear that we do not scold her for pooping in the house, in fact I feel bad like for some reason she felt that she didn't have the outside option. We always praise when she goes outside. Am I missing something?
